Dear Abi O'Brien

I am presenting this letter as a written notice that there was a work-related accident on the 25th of September at approximately 3:45 pm at Carmen road Burger King. This involved a worker, Ryan Timithy, who slipped over a spill of hot grease during rush hour. He regained consciousness after passing out for 2 minutes from impact. 3 witnesses saw the fall, those listed below. Immediately after the incident, the witnesses set medical aid for Ryan and were then sent to the hospital as soon as possible. One of the witnesses filed an injury at work report for Ryan as he was sent away. He was released from work later that day and was given 2 days' leave to recover.

Yours sincerely

Samantha Whitcombe
